The consultation becomes the file note.
Consent is taken first, every time. What was discussed, what was advised and what was agreed land on the matter while they are still accurate.
How a consultation becomes a record
Consent is taken before recording starts, every time, and the control stays disabled until it has been. What was discussed, what was advised and what was agreed become a drafted file note on the matter while they are still accurate. An adviser reviews that note before it lands on the file.

Advice is attributable

You review before it lands
cl. 1Nothing is filed until a licensed adviser has read it and signed it off.
